Creating conversations, dying well takes planning (One Croydon's training - part 3)

Wednesday 28 April 2021
10:00 to 13:00

This past year has taught us these conversations and plans are needed more than ever and this FREE training by St Christopher's Hospice will enable delegates to discover how to be more effective in extending conversations with their patients, family or social networks, in supporting the diverse population of Croydon, to achieve the best end of life possible (however far into the future that might be).

The course is delivered by experienced trainers and will be delivered in four parts on Wednesdays 14,21,28 April and 5 May 2021 from 10am-1pm. By attending these training sessions delegates will understand:

  • The Roadmap of holistic planning for End of Life
  • Develop a person focused plan to include My CMC and ACP
  • How to access ongoing individual support
