Job Title:  Personal Independence Coordinator & Reablement Link Service Manager

Salary:  £35,350 pa
Hours:   35 hours per week

Role:
Looking to recruit an experienced Manger to lead the team of Personal Independence Coordinators (PICs) and Reablement Link Workers. The PICs and Reablement Link Workers work alongside health and care professionals, adopting a multidisciplinary approach to working with older people who have long term health and social care needs. Most importantly, they ensure that care planning is influenced by the wishes identified by individuals, so that independence can be either improved or maintained.

Role:
PICs work alongside health and care professionals, adopting a multidisciplinary approach to working with people who have long term health and social care needs. The PICs provide critical links between formal health and social care services and the wider community support networks. AGE UK have adapted ways of working in response to COVID, helping older people to access technology and connect in new and exciting ways.  They are looking for an approach to working with people that ensures that care and support planning is influenced by the wishes identified by those individuals to improve their health and well being.
