Salary:       £25,710 fte

Role:
The PICs work alongside health and care professionals, adopting a multidisciplinary approach to working with older people who have long term health and social care needs. The PICs provide critical links between formal health and social care services and the wider community support networks. Most importantly, they are looking for an approach to working with people that ensures that care
planning is influenced by the wishes identified by those individuals so that independence can be either improved or maintained. The PICs ensure that people have a stronger voice in relation to issues that affect their lives. Role:
Looking to recruit a support worker as part of a new service in Croydon, known as Personal Independence Coordinators (PICs). The PICs work alongside health and care professionals, adopting a multidisciplinary approach to working with older people who have long term health and social care needs. Most importantly, they ensure that care planning is influenced by the wishes identified by individuals, so that independence can be either improved or maintained.  The Personal Independence Coordinator Support Worker will perform a key role within the team, providing time limited support to older people and supporting the case load of the Personal Independence Coordinator (PIC) team.

Job Title: PIC & Reablement Link Service Manager
Salary:     £35,000 FTE
Hours:     35 hours per week, fixed term for 9 months maternity cover, with an option to extend to 12 months

Role:
This is an exciting opportunity to lead pioneering programmes which are at the heart of Croydon’s New Model of Care, driving preventative, holistic, people-centred care. Looking to recruit an experienced Manger to lead the team of Personal Independence Coordinators (PICs) and Reablement Link Workers. The PICs and Reablement Link Workers work alongside health and care professionals, adopting a multidisciplinary approach to working with older people who have long term health and social care needs. Most importantly, they ensure that care planning is influenced by the wishes identified by individuals, so that independence can be either improved or maintained. The Service Manager will be responsible for the operational and strategic management of this project, ensuring the delivery of the programme on time and within budget.  Direct line management of 4 Team Leaders with a service staff team of 23 will need strong team building and personnel management skills.
