Tackling loneliness and isolation in older people

Compassionate Neighbours tackles the loneliness and isolation many people feel towards the end of life. 
 
The programme trains local people to become compassionate neighbours then match them with someone in their community who is in the last years of life due to age (85+) or illness (palliative condition).
 
Through weekly visits either at home or out in public, the pair build an organic, reciprocal, neighbourly relationship, increasing connection, confidence and belonging, and boosting both people's wellbeing. 
 
Compassionate Neighbours in South West London is delivered by a number of hospices, each covering a different catchment area. The details of each project and the areas they cover can be found below:
