SWL ICS Prevention Delivery Group (recruiting)

Aim: to deliver the preventive initiatives we can do at scale across South West London and to work with Place on their prevention work in local health and care plans. By Prevention, we mean primary prevention (prevent poor health), secondary prevention (reduce the need for treatment) and tertiary prevention (delay the need for care).

Agreed areas of focus:

  • To deliver the aims of the ICP strategy and monitor the progress being made on embedding prevention:
    • Developing a whole-system approach to healthy weight and reducing obesity
    • Maximising the ability of the voluntary and community sector to support people to lead healthier lifestyles
    • Developing personalised self-care for people with long-term conditions
  • To deliver the aims of the Joint Forward Plan on prevention:
    • Deepening our understanding of our population and health inequalities
    • Developing a healthy lifestyles pathway across SWL
    • Protecting SWL population from communicable diseases and environmental threats
    • Increasing support for the prevention and early diagnosis of chronic conditions
  • To use population health management to improve population health and reduce health inequalities.
Meetings: monthly, last Thursday of the month, Teams meeting.
 

Membership: a small number of core members: Director of Public Health, London Borough of Kingston (SRO for ICP Prevention workstream) [Co-Chair]; Director of Health Improvement (ICB)/SWL Public Health Consultant, South London Partnership (Lead for ICP Prevention workstream) [Co-chair]; Head of Transformation: Long Term Conditions & Population Health Research Lead (will be Head of Health Improvement); ICB Prevention & LTC managers; Place Leads; Local public health representatives.

What we are looking for: A strategic minded CEO/Director/Founder whose voluntary sector organisation works ideally across more than one borough in South West London (Merton, Croydon, Wandsworth, Sutton, Richmond and Kingston). The position might be of interest to a sport VCSE group given the focus on reducing weight and leading a healthier life style. The person needs to be willing to represent the wider voluntary sector rather than their own organisation and, to do so, to liaise with other organisations through and feed back to the SWL VCSE Alliance Director. They need to regularly attend local VCSE Forums and networks and be well connected with other voluntary sector organisations and their local infrastructure organisation.
